Pseudoaneurysms of the internal carotid artery (ICA) may develop as a late consequence of trauma and dissection.1'2 Although often considered a "benign" complication, pseudoaneurysms may present additional morbidity and management considerations.A 29-year-old woman presented with aphasia and dense right hemiparesis. Laboratory studies were remarkable only for positive phencydidine and tetrahydrocannabinol toxicology studies. Cigarette smoking was her only vascular risk factor. MRI revealed several acute infarctions throughout the left frontal, temporal and parietal lobes. Magnetic resonance angiography (MRA) revealed moderate narrowing of the origin of the left ICA and an eccentric intraluminal filling defect (Fig. 1A and B). Four vessel cerebral angiography revealed moderate focal stenosis at the origin of the left ICA and a mass-like intraluminal filling defect in the left carotid bulb (Fig. 1C). The right carotid artery, vertebrobasilar system and intracranial arteries exhibited no abnormalities.
