Sawyer's editorial in the September 2008 Journal of Paediatrics and Child Health discusses a number of general issues related to the care of adolescents and young adults with chronic illness. We agree with her call for comprehensive data collection to enhance the planning process. Our exploratory data collection exercise was a starting point which has now evolved into a statewide transition program.It is regrettable that the editorial suggests that, except in specific illness categories, there is little evidence of hospital wide or regional planning for young people transitioning to adult health care. There has, at least in New South Wales (NSW) and Western Australia (WA), been substantial progress in developing statewide collaborative approaches through the Greater Metropolitan Clinical taskforce (GMCT) Transition Network funded through NSW Health and through the WA Department of Health2.
