Sirs: One of the unique clinical signs of ocular myasthenia gravis (MG) is that saccade velocities remain unimpaired despite a reduced range of movement. In addition, small amplitude saccades may be hypermetric with high velocities appearing clinically as "quiver" movements which are thought to be characteristic of MG. Saccades may be abnormal due to neuromuscular fatigue with intra-saccadic slowing or selective involvement of specific extraocular muscle fibers. The latter hypothesis states that muscle fibers generating high speed movements (pale twitch fibers) maybe relatively spared, while muscle fibers responsible for maintenance of eccentric gaze (non-twitch fibers) are severely affected . Using phase-plane analysis, the conjugacyof the binocular initial components of saccades in MG patients with pseudo-internuclear ophthalmoplegia has recently been taken as evidence for selective sparing of pale global fibers in MG.
