Mr Gomez, together with his wife and 10-year-old daughter, sat timidly in my office. The family had traveled more than 1000 miles to a major cancer center seeking therapy. Mr Gomez was 40 years old and had been feeling tired over the past 2 years but had attributed the weariness to stress. He continued to work full time as an engineer but noted increasing difficulty with motivating himself to do routine chores, such as mowing the lawn. Although his malaise had worsened during the last year, he was a stoic individual and did not mention the fatigue to anyone. Six weeks before his visit, he developed fever and sore throat Initially, his family physician had thought that he had the flu. Physical examination revealed a more ominous sign, however-his spleen was enormously enlarged, reaching down to the pelvic brim. A hemogram showed shockingly low values: white blood cell count, 1.4 x 109/L; absolute neutrophil count, 0.1 x 109/L; platelet count, 20.0 x 109/L; and hemoglobin, 4.5 g/dL.
