Clinjicians who look after patients with venous thromboembo-lism may feel moderately well informed about the associations with, and complications of, deep vein thrombosis (DVT) affecting the leg. Management of lower limb DVT has been guided by several high-quality randomized studies, which have compared the risks of recurrence following varying durations of anticoagulation after a first episode [1-3], and by studies that have assessed the role of history-taking, thrombophilia testing, D-dimer measurement, and assessment of residual clot in predicting recurrence [4-6]. In this issue of the journal, Flinterman et al. [7] review the available data relating to upper extremity DVT (UEDVT), and Lechner et al. [8] present a comparison of the associations with and outcomes of UEDVT with lower extremity DVT (LEDVT) from a large prospective study of patients with an objectively proven first episode of venous thromboembolism.
