We have read with great interest the article by Caricato et al (1) on the usefulness of transcranial echography in patients with decompressive craniectomy, and we would like to congratulate them for their work. We think that these data are very important for particular situations like untransportable patients or during air medical evacuations. With the increasing number of battlefield casualties in these last 10 yrs, we have to transport an increasing number of patients with severe traumatic brain injuries. Portable echography is the main multipurpose tool available for patients "global" monitoring during this high-risk period (abdominal, cardiac, pulmonary, vascular echography, and particularly for traumatic brain injury transcranial Doppler).
