This article focuses on the evolution and rationale for chemotherapy for children and young adults with primary bone tumors; OS, ES, and PBL. Currently, there are no standard therapies for metastatic OS or ES and clinicians must either use the standard localized approaches or enroll in several new clinical trials for high-risk disease. CS, the third most common malignant bone tumor in patients younger than 50 years is a surgically managed disease and is not routinely treated with chemotherapy. As previously emphasized, participation in a clinical research trial, therapeutic, biological, registry, or cancer control, is considered the goal for all patients and is in fact, the "standard of care" for children with cancer.
