A few years ago the first reports were published of decom-pressive trepanation to treat sinus vein thrombosis with malignant cerebral oedema [6, 7, 10]. Several studies have shown good outcomes, even in patients with severe clinical conditions [2, 5, 8, 9]. These results encourage the use of decompressive surgery as a life-saving procedure in selected patients. We report for the first time perfusion of the cortical veins before and after decompressive trepanation as seen on computed tomography (CT). These pathophysiological observations may explain in part the often good functional outcome after surgery.
