Immune reconstitution inflammatory syndrome (IRIS) is a pathological inflammatory reaction that may occur after HAART initiation in AIDS patients and target either previously treated or subclinical infections. We report the succession of an unusual acute presentation of tuberculous meningitis unmasked by antiretroviral treatment, corresponding to the first such case of IRIS, immediately followed by the onset of a severe 'paradoxical reaction' consisting of cerebral tuberculornas.
