摘要

HLA-HPA-immunized patients require HLA-HPA-matched platelets. We supplied HLA-HPA-matched platelets to 14 patients in which HPA of their HLA-matched donors were typed by the MPHA method. Since 1996 we have typed HPA-1, -2, -3, -4, -5, -6 of 1,705 donors (20.7%) by DNA-typing (ELMA) from 8,226 HLA-typed donors to establish an HLA-HPA-typed donor pool.Patients with anti-HPA-lb, -2b, -3a, -5b, and -Naka received HPA-matched donors in 95.2%, 75.8%, 17.7%, 91.7% and 10.0% of HPA-typed donors from their HLA-matched donors, respectively.Fourteen patients will fully receive HLA-HPA-matched donors from the present HLA-HPA-typed donor pool unless they frequently require HLA-HPA-matched platelets in long-term treatment.HLA-HPA-matched donors are determined according to the following characteristics: 1) frequency and term of patients' HLA-HPA-matched platelet transfusion 2) antigen frequency of patients' own HLA and patients' acceptable HLA 3) donors' HPA frequency.We consider we were able to establish an HLA-HPA-typed donor pool for most patients. However, if patients are not able to fully receive HLA-HPA-matched donors from the present donor pool, it will be necessary to type HPA of their HLA-matched donors more efficiently, or to cooperate with other Red Cross Blood Centers in order to supply HLA-HPA-matched platelets smoothly.
