It sounds like something Socrates would ask: What is value? Twenty-four hundred years later, those who would define what guides the rational use of healthcare struggle mightily to answer that question. Value-based benefit design (VBBD) is an approach to define, if not achieve, value. Intuitively, VBBD makes sense: Lowering access barri-ers to services that keep people healthy and productive not only reduces underuse of high-value services, it also addresses the overuse of low-value services that existing benefit designs encourage. And, if done right, you might improve health sta-tus, save money, and align stakeholder interests along the way. In a recent Health Affairs article (Robinson 2010), James C. Robinson, PhD, MPH, the Leonard D. Schaeffer Professor of Health Economics at University of California-Berkeley School of Public Health, suggested that VBBD has passed the proof-of-concept phase. For years, VBBD has been applied with some success to the question of whether people with diabetes or asthma should have easier access to maintenance medications. But -like finding an answer to anything Socrates asked - defining value to apply the concept to benefit designs for biologics is tough.
