I read with interest the article by Abdulatif et al. [1] on the use of intra-operative magnesium to prevent postoperative agitation in children aged 4-7 years, undergoing adenotonsillectomy. It suggests an interesting and novel way to manage a common, and often under-reported, problem.I would like to draw attention to the role of pain as a possible confounding factor in this study. Pain is a major risk factor for postoperative agitation [2] and although the role of magnesium as a co-analgesic is controversial, two recent meta-analyses concluded that systemic administration of peri-operative magnesium reduces postoperative pain and opi-oid consumption [3, 4].
