There is no standard treatment for patients with dry eye syndrome who find artificial tears inadequate. Non-drug treatments and steroid eye drops (used off-licence) are sometimes proposed. Eye drops containing 0.1% ciclosporin, an immunosuppressant, have been authorised in the European Union for patients with dry eye syndrome and severe keratitis (Ikervisdeg, Santen). This oil-in-water emulsion also contains cetal-konium chloride among its excipients. Clinical evaluation is mainly based on a randomised, double-blind trial that compared 0.1% ciclosporin eye drops with the vehicle alone (without ciclosporin) in 244 adults with dry eye syndrome and severe keratitis. After 6 months, there was no difference between the groups in terms of changes in symptoms or artificial tear use. These 0.1% ciclosporin eye drops have frequent ocular adverse effects, including pain, irritation and lacrimation. In clinical trials, ocular adverse effects were severe in 22% of patients using the ciclosporin eye drops, versus 10% of patients using the vehicle.The immuno-suppressive effects of ciclosporin put patients at an increased risk of eye infections and, possibly, ocular or periocular cancer. Simultaneous use of steroid-containing eye drops and ciclosporin eye drops can lead to additive immunosuppressive effects, further increasing the risk of infection.
