In a randomised trial in about 400 patients, low-dose colchicine did not have greater efficacy than naproxen in relieving the pain associated with gout attacks. Given its sometimes serious adverse effects, colchicine is an option of last resort. In a patient experiencing a gout attack, applying ice to the affected joint appears to alleviate the pain a little. When it seems worth adding a drug to relieve the pain, the first choice is between the two nonsteroidal anti-inflammatory drugs (NSAIDs) with the best harm-benefit balance: ibuprofen or naproxen. An oral corticosteroid has similar efficacy to an NSAID, but with different adverse effects. Colchicine, a cytotoxic drug with a narrow therapeutic index, is an option associated with a higher risk of serious adverse effects (1,2). Is colchicine more effective than an NSAID in relieving the pain associated with gout attacks? A non-blinded multicentre trial published in 2020 compared oral naproxen (a starting dose of 750 mg followed by 250 mg every 8 hours for 7 days) versus oral colchicine (0.5 mg every 8 hours for 4 days) in 399 adults experiencing a gout attack. At baseline, these patients (average age 59 years) had an average pain score of about 7 on a scale of 0 to 10 (3).
