摘要

The introduction of stapling devices greatly facilitates low rectal anastomosis. Despite widespread application of this innovation, there is the legitimate concern about implanting exfoliated tumour cells by the staples. Theoretically, on-tab]e rectal stump washout with a clamp applied below the tumour before resecting the specimen can minimize the risks of local recurrence brought about by implantation of viable tumour cells. However, to date, routine use of rectal lavage during anterior resection or total mesorectal excision for cancer of the rectum has not been established as a universal practice. Notably, the author of a major colorectal surgery textbook published in 2005, based on a lack of hard evidence in support of its use in the contemporary published work, stated categorically that 'I... do not use it (lavage)' . Perhaps, the following case could be a stark reminder of the real risk of staple-line implantation.
