A 6-year-old human immunodeficiency virus-negative child presented with fever and weight loss. The chest X-ray revealed hilar lymphadenopathy and there was associated minor abdominal lymphadenopathy on abdominal ultrasound. The patient was commenced on empirical anti-tuberculosis chemotherapy due to a strong contact history. Despite good compliance, the patient deteriorated and 3 months later had ongoing fever, generalized lymphadenopathy, mild hepatosplenomegaly and ascites.
