A 66-year-old man presented with haematuria and flank pain. Computed tomography (CT) imaging showed a 9.5 x 8.2 cm mass in the right kidney (asterisk, left), consistent with a renal cell carcinoma. He was also significantly pancytopenic; bone marrow examination revealed an extensive infiltrate of myeloblasts (CD34+, CD117+, myeloperoxidase+) and trilineage dysplasia. Cytogenetic analysis demonstrated three related cell lines all with monosomy 7 and additional abnormalities of trisomy 13, t(2;16)(qll; q11) and idic(22), establishing the diagnosis of acute myeloid leukaemia (AML) with myelodysplasia-related changes. In view of these poor prognostic features, treatment of the AML was prioritized and he received standard daunorubicin plus cytarabine induction chemotherapy. However, he achieved complete remission (CR) only after salvage with FLAG-Ida (fludarabine, cytarabine, idarubicin and lenograstim). This was consolidated with a matched, unrelated-donor allogeneic stem cell transplant (SCT) with conditioning according to the Seattle protocol. Interval CT imaging prior to the SCT demonstrated no significant reduction in size of the renal mass.
