摘要

Objective: To report the clinical effect of applying the flap pedicled with the ascending portion of the supracar-pal cutaneous branch of ulnar artery to repair skin defect on fingers. Methods : To design flaps on the distal end of forearm along the interspace of ulnar bone and musculus flexor carpi-ulnaris where was 4-9 cm up the wrist. The area of the biggest flap was 4 x 3 cm and the area of the smallest flap was 2.5 x 1.5 cm. The area of the biggest repaired raw surface was 3.5 x 2.5 cm, and the area of the smallest repaired raw surface was 2 x 1.5 cm. The ascending portion of the supracarpal cutaneous branch of ulnar artery provided the blood supply, the vein of the flap was the superficial vein on the u1 -saris forearm, and the sensory nerve of the flap was the end branch of the medial antebrachial cutaneous nerve.Results: The flap pedicled with the ascending portion of the supracarpal cutaneous branch of ulnar artery was transplanted to repair 16 cases of small skin defects on the fingers. All flaps were aesthetically acceptable postoperatively, and had good sensory recovery. In all 16 cases, there were four cases of S4, nine cases of S3, three cases of S3 and no case which was lower than S3 (SOS2).The rate of cases which were higher than S3 (S3 and S4) was 81.25%.Conclusion: Using a flap pedicled with the ascending portion of the supracarpal cutaneous branch of ulnar artery to repair skin defect on fingers is a feasible solution fortreatment of finger injury. Because this flap has many advantages, such as preserving the main vascellum, the injury to the donor site is slight, and the shape and sensation of wounded finger after it repair are satisfactory.Key words: Ulnar artery; Surgical flap; Transplantation; Finger; Microsurgical technique
机译:目的:报道用带蒂的尺动脉上-掌皮支上升段蒂皮瓣修复手指皮肤缺损的临床效果。方法:沿尺骨与腕腕屈肌腕间尺骨间间隙设计前臂远端的皮瓣。最大皮瓣的面积为4 x 3厘米,最小皮瓣的面积为2.5 x 1.5厘米。修复后的最大原始表面的面积为3.5 x 2.5厘米,修复后的最小原始表面的面积为2 x 1.5厘米。尺动脉掌上皮支的上升部分提供了血液供应,皮瓣的静脉是前臂u1-莎丽皮的浅静脉,皮瓣的感觉神经是前臂上皮神经的末端分支。结果:移植有蒂的尺骨腕上皮支分支的蒂皮瓣修复16例手指小皮肤缺损。术后所有皮瓣在美学上均可接受,并且感觉恢复良好。在全部16例中,有S4例4例,S3例9例,S3例3例,且没有低于S3(SOS2)的病例,高于S3的病例(S3和S4)的发生率为81.25%结论:用带蒂的尺动脉掌上皮肤分支上升部分的皮瓣修复手指皮肤缺损是治疗手指损伤的可行方法。由于该皮瓣具有许多优点,如保留主血管,对供体部位的损伤很小,修复后受伤的手指的形状和感觉令人满意。手术皮瓣移植;手指;显微外科技术
