I read with interest the article by de Vries et al. concerning the management of rare and challenging subtrochanteric nonunions.1 Out of 33 subtrochanteric nonunions in their series, a blade plate was initially used for fracture fixation in 10 cases. The authors state that a blade plate is not an appropriate device for primary treatment of subtrochanteric fractures. When the same blade plate device was used for more difficult and complicated nonunions, they achieved a high success rate (32 out of 33 cases). It would be interesting to analyse what errors were made during the primary surgery that led to nonunion in these cases. This would help refine the plating technique and prevent a future complication of nonunion.
