Dear Sir, Mereddy et al. have provided an interesting study of their clinical results using PFNA with unstable proximal femoral fractures. Their description of operative techniques, fracture classification, fracture reduction and post-operative radiographic measurements were exemplary. I want to congratulate Mereddy et al. on their excellent results and expertly presented article. Over the last two years, I have regularly used PFNA for unstable proximal femoral fractures. However, compared with the study of Mereddy et al. who used PFNA long (79%) more frequently than PFNA standard (26%), I use PFNA standard in 90% of my cases, the exception is when the proximal femur fracture includes the metaphyseal or diaphyseal femur. Therefore I believe that using PFNA is the best available method for treating an unstable proximal femur fracture, and that it is the ideal method to succeed in osteosintesis of the proximal femur when DHS fixation fails as the authors mentioned. I have also used PFNA in cases following Gamma nail complications. I concur with the data which Mereddy et al. present in their introduction regarding the Gamma nail in connection with complications.
