We read this article with interest. It is stated that the hook of the plate acts as a lever to elevate the lateral fragment. In fact, in this case it is the medial fragment that is unstable and displaced superiorly, hence the plate acts as a depressor that brings the medial fragment down.It is generally accepted that Neer's type ill fractures should be managed non-operatively. One third (10 out of 31) of the patients in this series consists of those with Neer's type III fractures. Nevertheless, justification for this choice was not contemplated either in the Material and methods nor in the Discussion. It would be very valuable if the authors would further explain this point.
