It has been 81 years since the publication of one of last century's best-loved books, The Story of San Michele1 by the Swedish physician Axel Munthe (Fig. 1). This enchanting, semi-autobiographic tale recalls the altruistic ideals and artistic dreams of a benevolent physician working in southern Italy during the late-1800s. The book was an instant bestseller and has been translated in over forty languages, so I was dismayed when a straw-poll of colleagues taken during my postgraduate fellowship in the United States revealed none had heard of it! I therefore felt compelled to offer a timely reprise of Dr Munthe's extraordinary life and legacy (for a detailed biography see Jangfeld2 and the Villa San Michele website at http:// www.villasanmichele.eu).
