摘要

A 56-year-old male, with a known history of mitochondrial myopathy diagnosed 12 years previously, presented with a 3-day history of abdominal distension and absolute constipation. The abdominal radiograph showed a markedly distended large bowel loop; decompression with a flatus tube resulted in symptomatic improvement, and the patient was discharged after 4 days. The patient presented, 10 weeks later, with a second and similar episode. A working diagnosis of sigmoid volvulus was made at the time, and after insertion of two further flatus tubes the patient improved symptomatically with a repeat radiograph demonstrating decompression of the large bowel. Within 2 days, however, the symptoms recurred, and the patient proceeded to have a laparoscopic sigmoid colectomy. Histological analysis of the sigmoid colon showed focal dilatation of the bowel wall without evidence of ischaemia.
