We thank Dr Moorthy and Dr Rajsekharan for showing interest in our article and giving their comments. We agree that in expert hands, a minimally invasive procedure via stereotactic route may be a viable option in brainstem lesions. However, for the vast majority of neurosurgeons, open surgery offers the safest and easiest way to approach the brainstem. It is also pertinent to note that tubercular pus can be very thick making needle aspiration difficult, if not impossible. It may therefore be prudent to surgically evacuate the pus so as to ensure complete decompression of the abscess as well as to remove compression on the brainstem.
