A 78-year-old man with severe symptomatic degenerative aortic stenosis and high-surgical risk was referred for transcatheter aortic valve implantation (TAVI). Routine two-dimensional transoesophageal echocardiography (TEE) performed during TAVI procedure revealed a possible cleft or slit at the mid-portion (P2) of the posterior mitral leaflet and a mild central mitral regurgitation.
