Ischemic priapism can result in erectile dysfunction if unresponsive to treatment. A recent study advocates immediate penile prosthesis implantation in patients with treatment-refractory ischemic priapism to avoid loss of penile length and to facilitate insertion of the prosthesis. Such a protocol may well become the standard of care in future.Ischemic priapism is a persistent non-sexual erection associated with absent or low blood flow and abnormal cavernous blood gases (pH <7.25, P_(O2) <30 mmHg, and P_(CO2) >60mmHg). The penis is abnormally rigid and the erection is painful; color duplex ultrasonography shows minimal or absent blood flow in the cavernosal arteries. Ischemic priapism that is not successfully reversed often results in cavernosal smooth muscle death, fibrosis, and erectile dysfunction.
