The article in this issue by Corrigan, Desnick, Bentov, Marshall, and Rosenblatt on the perceptions of medical students to patients with complaints of pain in primary care settings is important and is a concern. This study reports on a qualitative analysis of the reflections of first year medical students who are participating in primary care rotations in rural and underserved settings. In their written reflections, over half of the students referenced their reactions to patients complaining of pain, and almost all of those students included concerns about chronic pain patients in their reflections. The investigators divided the student reflections on chronic pain into positive, negative, and neutral. Seventy-five percent of the students who offered reflections on pain patients described negative perceptions. Negative comments were significant and powerful, often highlighting the students' problems in dealing with ambiguity. Many had problems with the concept of pain in the absence of significant physical findings, and many had real concerns about the use of opioid therapy in the treatment of chronic pain of nonma-lignant origin. As noted in the article, "They (the students) questioned whether chronic pain patients were being untruthful or manipulative. The idea that these patients still needed to be cared for challenged students' own beliefs about what is 'true' pain."
