Comparison of the Quality of Life of ESRD (End-Stage Renal Disease) Patients

The study examined the quality of life of patients on alternative therapies for end-stage renal disease. Four categories of patients were compared: transplant recipients from the 1970's, transplant recipients from the 1980 to 1984 time period, in-center hemodialysis patients, and patients on continuous ambulatory peritoneal dialysis (CAPD). The transplant patients from 1980 to 1984 were randomized either to a cyclosporine immunosuppressive regimen or to a conventional regimen (azathioprine). Survey questionnaires were administered containing measure of physical, emotional and social well-being, vocational rehabilitation, and sexual adjustment. Case-mix differences were controlled, insofar as possible, with an Analysis of Covariance; adjusted means were compared. Findings indicate that the quality of life for successful transplant patients exceeds that of both dialysis groups for almost all variables. There is evidence that CAPD patients achieve a higher quality of life than in-center hemodialysis patients. Among transplant patients, patients randomized to cyclosporine, as compared to patients randomized to conventional immunosuppressive show significant advantages in physical, emotional and social well-being. In conclusion, these studies suggest that the effects of these expensive therapies should not be evaluated solely on medical grounds or in terms of therapeutic success narrowly defined. Life-quality, as well as life-extension is important to consider.
