The objective of the study was to investigate factors that were associated with the utilization of services in the treatment of hypertension and thus with the costs of care. Data were obtained from one of the clinical centers (Boston) participating in the Hypertension Detection and Follow-Up Program (HDFP). HDFP, sponsored by the National Heart, Lung and Blood Institute, was a 5-year randomized clinical trial of comparative approaches to the treatment of hypertension. This trial involved 10,940 individuals who were randomly assigned to two groups. The Stepped Care (SC) group received graded drug therapy for hypertension according to a protocol and the Referred Care (RC) group was referred to community sources for conventional care. The Boston Clinical Center had 750 participants (371 in the SC group). The hypothesis was that the amount of care that would be required by each SC participant over the 5-year course of the trial would reflect clinically measurable characteristics of the individual that would be identifiable upon their entry into the program.
