摘要

The purpose of this project was to demonstrate the implementation of managed carehealth plans in rural areas by having university-based research and technical experts working with local communities. During the five years of the project the external environment, including Medicaid and Medicare policies, changed from pressure to adopt managed care plans to departure of managed care plans from service areas. The emphasis in the project shifted to one of helping local health care providers be more influential regarding the terms of contracts they signed and in improving the health of residents of their communities. Through direct participation in local rural provider networks, assistance in planning and market analysis, and educational activities, the project team helped local providers gain greater control of health care financing. With the help of external consultants, including special symposia, rural networks developed plans for 'roll out' of managed care, which were then used to strengthen negotiations with existing insurance carriers and to develop local plans in the two Nebraska sites. At the end of project, local providers were in a much stronger position to participate in managed care plans.
