    • 摘要: 目的:探究BAL与NIPPV用于COPD合并Ⅱ型呼吸衰竭患者治疗中对肺功能及睡眠的影响.方法:选取2016年3月至2018年8月重庆市綦江区人民医院收治的COPD合并Ⅱ型呼吸衰竭患者72例作为研究对象,随机分为观察组和对照组,每组36例.对照组采用NIPPV进行治疗,观察组采用BAL与NIPPV进行治疗.结果:观察组治疗后的FEV1、FVC以及FEV1占预计值的百分比均明显优于对照组;观察组PSQI评分明显低于对照组,差异均有统计学意义(均P<0.05).结论:将BAL与NIPPV用于COPD合并Ⅱ型呼吸衰竭患者的临床治疗当中,患者临床症状改善明显,睡眠质量状况也有所改善,值得临床上应用推广.

    • 摘要: 目的:探讨支气管肺泡灌洗(BAL)应用于重症肺部感染患者治疗中的效果.方法:选取我院2019年1月—2020年6月所收治重症肺部感染患者80例,随机分为研究组和对照组.对照组40例,予以常规抗感染治疗.研究组40例,在常规抗感染治疗基础上加用BAL.比较两组患者临床表现指标、SpO2及疗效.结果:研究组中,感染窗控制时间、机械通气时间、住院时间分别为(5.56±1.88)、(10.08±3.19)、(19.15±3.26)d,均低于对照组,差异具有统计学意义(P<0.05).治疗后,研究组SpO2为(96.01±3.29)%,高于对照组的(90.29±3.20)%差异具有统计学意义(P<0.05).研究组总有效率为95%,高于对照组的75%,差异具有统计学意义(P<0.05).结论:BAL有利于提升重症肺部感染患者治疗效率、改善血气情况,对临床疗效提升具有积极意义,值得临床应用.

    • 摘要: 目的 探讨强化排痰方案辅助BAL对行机械通气重症肺炎患者呼吸功能、生命体征及死亡率的影响.方法 选取我院2015年8月-2017年8月收治行机械通气重症肺炎患者共110例,以随机数字表法分为对照组(55例)和观察组(55例),分别给予BAL单用和在此基础上加用强化排痰方案治疗;比较两组患者临床疗效、治疗前后呼吸功能指标水平、生命体征指标水平、机械通气时间、ICU住院时间及随访28d死亡率.结果 观察组患者治疗总有效率显著高于对照组(P0.05);观察组患者机械通气时间和ICU住院时间均显著短于对照组(P0.05).结论 强化排痰方案辅助BAL治疗行机械通气重症肺炎可有效控制感染症状,改善呼吸功能,加快病情康复进程,且未影响生命体征.

    • 摘要: 目的 分析BAL在呼吸系统有关疾病中的临床应用.方法 回顾我院自2016年6月开始开展这一技术以来至今已应用的31例病例,统计该技术应用的成功率与副反应、检查情况及治疗结果;结果成功率为100%,副作用是咳嗽加剧,予局部喷入利多卡因即可缓解;BAL检查阳性13例,阳性率41.9%;2例呼吸道感染患者予以支气管镜下BAL治疗后均好转出院.结论 BAL可靠性高,安全性好,减轻患者痛苦,能指导临床诊疗,值得推广.

    • 摘要: 127例机械通气下肺部感染随机分为无痛灌洗组(治疗组)和对照组,两组均给予抗感染、常规吸痰,机械通气等支持对症治疗,无痛灌洗组予镇静,镇痛下经纤支镜肺泡灌洗。对两组进行疗效比较。两组在病原菌培养阳性率,血气改善,机械通气时间,肺部炎症基本吸收时间,抗生素使用时间,有效率,治愈率、住院时间等比较,差异有统计学意义(P<0.05)。并且操作术中躁动、恶心、呕吐、呛咳等不良反应发生率低,术中仅有5例出现轻度的一过性低氧血症,2例发生窦性心动过速,未发生严重心律失常及血压明显下降等。无痛BAL技术操作较简单,创伤小,可反复进行,在重症肺炎的诊疗中能更准确明确病原菌并指导治疗,改善有效通气,同时又降低了患者不适反应发生率,具有积极推广应用的价值。

    • 摘要: 目的:探讨支气管肺泡灌洗(BAL)联合无创正压通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)合并II型呼吸衰竭的临床效果。方法:选取我中心2011年1月至2013年1月间出诊送到同一家医院且收治的COPD合并II型呼吸衰竭的患者86例,随机分为观察组与对照组,观察组在常规治疗的基础上应用BAL联合NIPPV 治疗,对照组单独采用NIPPV 治疗,对比两组患者血气指标、心率、平均动脉压、APACHE II评分变化及不良反应发生情况。结果:两组患者治疗后PaCO2均较治疗前显著下降,治疗前后比较差异有统计学意义(P<0.05);观察组患者治疗后PaCO2下降较对照组更加迅速,组间比较差异有统计学意义( P<0.05);且观察组患者在治疗后24h、48h PaO2均显著高于对照组,组间比较差异有统计学意义( P<0.05);且观察组患者气管插管率显著低于对照组,组间比较差异有统计学意义( P<0.05);两组患者心率、平均动脉压治疗24h后比较无统计学差异( P>0.05)。结论:BAL联合NIPPV治疗COPD合并Ⅱ型呼吸衰竭能有效的降低患者PaCO2,提高PaO2,可迅速缓解患者呼吸衰竭症状,是治疗COPD合并II型呼吸衰竭的有效治疗方案。%Objective:To investigate the effect of bronchoalveolar lavage ( BAL ) joint non-invasive positive pressure ventilation (NIPPV) in treatment of chronic obstructive pulmonary disease (COPD) com-bined with typeⅡrespiratory failure. Method:A total of 106 patients with COPD combined with type Ⅱre-spiratory failure admitted in our hospital from Jan. 2011 to Jan. 2013 were randomly divided into observation group and control group. Observation group was treated with BAL on the basis of NIPPV treatment, control group adopted NIPPV treatment alone, compared two groups of patients with blood gas index, heart rate, breathing rate, APACHE II score changes and adverse reactions occurrence. Result:After treatment, PaCO2 in two groups was decreased with statistically significant difference (P0. 05). Conclusion:Therapy of the BAL joint NIPPV in treating COPD combined with type Ⅱrespiratory failure can more effectively reduce PaCO 2, improve PaO2, can rapidly alleviate symptoms of pa-tients with respiratory failure, so this is an effective therapy in the treatment of COPD with type II respiratory failure.     • 摘要: Objective To investigate the effect and safety of broncho alveolar lavage in the treatment of COPD patients who were received mechanical ventilation. Methods 90 COPD patients were randomly divided into the observation group and the control group. On the basis of conventional therapy, the observation group received broncho alveolar lavage, and the control group received normal lavage. The effect and safety were compared between the two groups. Results The improvement of airway resistance, time of mechanical ventilation and antibiotics use, achievement ratio of off-line was more pronounced in the observation group than in the control group with significant difference ( P <0. 05 ). The indexes of PaO2 and PaCO2 were superior in the observation group than those of the control group with significant difference ( P <0. 05 ). There were 2 cases with ephemeral hypoxemia, 1 case with sinus tachycardia and no serious cardiac arrhythmia and dramatic decline of blood pressure reported. Conclusion Broncho alveolar lavage has the advantages of easy-control, mini trauma and repeatability. It can identify the pathogenic bacteria and improve the effective ventilation, so it is worth to be applied and promoted in clinical practice.%目的 探讨纤支镜支气管肺泡灌洗(BAL)在COPD机械通气患者中的应用效果与安全性.方法 选取在我院住院COPD患者90例,分为观察组和对照组,在综合治疗的基础上观察组给予纤支镜BAL,对照组给予普通灌洗,比较两者的治疗效果及安全性.结果 观察组在气道阻力、机械通气时间、脱机成功率及抗生素应用时间方面明显优于对照组,(P<0.05).观察组PaO2、PaCO2指标明显优于对照组,(P<0.05).操作中有2例出现轻度的一过性低氧血症,1例发生窦性心动过速,未发生严重心律失常及血压明显下降等.结论 BAL技术操作较简单,创伤小,可反复进行,在COPD的诊疗中能明确病原菌并指导治疗,改善有效通气,具有很大的应用价值.

    • 摘要: Biphenotypic acute leukemia (BAL) is an uncommon clinical entity. It is a type of acute leukemia with features characteristic of both the myeloid and lymphoid lineages and for this reason is designated as mixed-lineage, hybrid or biphenotypic acute leukemia. As strict diagnostic criteria have only recently been established, the precise incidence among acute leukemia is uncertain, although it is likely to account for approximately less then 5% of all acute leukemia. BAL is now collectively considered as “mixed phenotype acute leukemia” (MPAL). We hereby report two cases of a rare disease, BAL from our institution in the light of morphology, cytochemistry, flow cytometry and review of literature regarding these cases are described.

    • 摘要: We report a case of exogenous lipoid pneumonia caused by inhalation of liquid paraffin in a fire eater young man. The patient was admitted to the Emergency Department because of acute retrosternal pain, dry cough and dyspnea. Chest radiograph showed multiple bilateral basal alveolar opacities associated to left pleural effusion;CT chest scan showed multiple bilateral areas of consolidation with cavity areas. BAL cytologic examination revealed abundant lipid-laden macrophages. Clinical symptoms resolved after corticosteroid treatment. Lipoid pneumonia is a rare and often underdiagnosed entity. The presence of lipid-laden macrophages in sputum or BAL fluid helps to confirm the diagnosis.
