In everyday clinical practice, when the patient's psychotic symptoms are reasonably under control, the clinician's challenge is to deal with concomitant psy-chopathology and psychosocial problems in the most appropriate way. Anxiety disorders can be diagnosed in up to 43% of patients with a diagnosis of schizophrenia and may considerably influence the quality of life of the patient and the carers and the prognosis of the psychotic disorder. A cognitive-behavioral approach based on psychoeducation and using the appropriate strategies (e.g., slow breathing and gradual exposure for panic disorder and agoraphobia, problem solving for general anxiety) can be sufficient to control the anxiety disorder without adding new medications. The clinical improvement of anxiety can lower the general level of stress and often leads to better control of the psychotic disorder and a decrease in disabilities, and can foster the collaborative alliance with the patient and the carers. A module on anxiety disorders for patients and their carers can be easily integrated into the clinical management of schizophrenia. Four clinical vignettes will illustrate specific issues, such as the diagnostic approach, integrating the intervention on anxiety with the treatment of schizophrenia, and "dosing" the approach to individual needs.
