Deep infiltrating endometriosis (DIE) is one of the most challenging problems in operative gynecology (KECKSTEIN et al. 2003). Despite that, reality shows that surgeons and patients alike are often not properly prepared if it comes to the surgical treatment of DIE. This lecture is addressed to our younger colleagues who want to start with the surgery for DIE.DIE is common. While the pelvic peritoneum and the ovaries are the most common manifestations of the disease, they are followed by the uterosacral ligaments, the rectovaginal septum, and the bowel (SHAW 1993). Let's talk about a situation known to every gynecologist: You perform a laparoscopy in a patient with dysmenorrhea and/or chronic abdominal discomfort - and you may be prepared for some peritoneal implants. However, you find severe DIE. And you are surprised.
