目的:分析恶性血液病脊柱骨髓浸润的脊柱MRI表现,探讨MRI对血液病及淋巴瘤骨髓脊柱浸润的诊断与鉴别诊断价值.方法:回顾2010年9月-2011年12月38例经骨髓穿刺或活检证实及PET-CT提示脊柱骨髓浸润患者的临床及脊柱MRI资料.观察脊椎骨髓MRI的信号、形态、分布、强化及周围软组织情况.结果:急性白血病患者21例,多发骨髓瘤7例,淋巴瘤骨浸润患者10例,T1WI信号均见局灶性、弥漫性或混杂性减低,35例STIR见范围大小不等的异常高信号.其中行增强扫描27例,25例脊椎增强扫描椎体见强化,2例未见强化.结论:脊柱MRI能很好地发现血液病脊柱骨髓浸润,对各种血液病骨髓浸润鉴别诊断有价值.%Objective: analyze the MRI representations of spiral marrow infiltration of malignant blood disease, and discuss the effect of JV1RI in diagnosis and identification of blood disease and lymphoma spiral marrow infiltration. Methods: review 38 cases of spiral marrow infiltration patients verified by marrow aspiration or biopsy and PET-CT prompt from Sep 2010 to Dec 2011, and data on spine MRI. Observe the signals, forms, distribution, strengthening and soft tissue of spiral marrow MRI. Results: the T1W1 signals of 21 acute leukemia patients, 7multiple myeloma patients and 10 lymphomatous infiltrate patients showed focal, diffuse or reduced hybridity, and there were abnormally high signals in different STIR scope in 35 cases. Enhanced scan was used in 27 cases, the centrum of 25 cases was enhanced and 2 remained the same. Conclusions: spine MRI can easily discover spiral marrow infiltration of blood disease and is effective for the diagnosis of spiral marrow infiltration of blood diseases.
