棘突间动态内固定装置治疗椎间盘突出症临床研究

         

摘要

目的 旨在通过对比研究Coflex棘突间动态固定系统植入术治疗腰椎间盘突出症的近期临床疗效、安全性及意义.方法 收集腰椎间盘突出症患者60例,将其随机分为单纯组和Coflex组,每组各30例.单纯组接受单纯椎间盘髓核摘除减压术,Coflex组摘除责任节段椎间盘髓核后于该节段棘突间植入Coflex钛合金材料固定.分别于术前及术后1、6、12个月行VAS评分、腰椎JOA评分及Oswestry功能障碍指数,同时行X线及CT检查并测量手术节段椎间盘背侧及腹侧高度、椎间孔间最大距离,棘突顶距及椎管面积等相关指标.结果 Coflex组的改善效果明显好于单纯组,术后12个月VAS评分、腰椎JOA评分及Oswestry功能障碍指数评定结果Coflex组与单纯组相比,差异有统计学意义(P < 0.05);术后12个月两组椎间盘背侧高度、椎间孔间最大距离、棘突顶距及椎管面积与各自术前对比,差异有统计学意义(P < 0.05),且两组术后12个月结果对比,差异有统计学意义(P < 0.05).结论 棘突间动态内固定装置治疗腰椎间盘突出症在近期临床疗效、防止腰椎退变、增加椎间盘背侧高度、增加椎间孔间最大距离、维持椎间隙高度及腰椎间盘突出症根性症状的缓解等方面具有积极作用.%Objective To analyze the short-term clinical efficacy, safety and significance of the Coflex interspinal dynamic fixation system in the treatment of lumbar disc herniation. Methods 60 cases of lumbar disc herniation were collected and randomly divided into simple group (30 cases) and Coflex group (30 cases). The simple group received simply remove the responsibility of discectomy decompression segment of intervertebral disc; and the Coflex group implanted Coflex titanium material between the spinous processes at the base of the simple group. All patients assessed by VAS, lumbar JOA score and the Oswestry Disability Index in preoperative and postoperative 1,6, 12 months. Through X-ray film and CT to measure surgical segmental dorsal disc height, the ventral disc height, intervertebral foramen between the maximum distance, spinous process zenith distance and spinal canal area. Results Coflex group improvement was significantly better than the simple group; the results of VAS, lumbar JOA score and the Oswestry Disability Index assessment was statistically significant between the Coflex group and simple group in postoperative 12 months (P < 0.05); the results of the surgical segmental dorsal disc height, intervertebral foramen between the maximum distance, spinous process zenith distance and spinal canal area assessment were statistically significant compared with the same group of preoperative in postoperative 12 months (P < 0.05), and it was statistically significant compared with two groups (P < 0.05). Conclusion The degenerative lumbar disc herniation by interspinal dynamic fixation system have a positive effect of term clinical efficacy, prevent lumbar degeneration, increase the height of disc dorsal, increase the maximum distance between the intervertebral foramen, maintain the height of the intervertebral space and relieve radicular symptoms of lumbar disc herniation, etc.
