重症监护病房多重耐药铜绿假单胞菌感染的耐药性分析

         

摘要

Objective To investigate the drug resistance of multi-drug resistant pseudomonas aeruginosa to commonly used antibiotics in intensive care unit (ICU) of hospital, in order to guide clinical rational drug use. Methods The bacterial strains were identified by Microscan-autoscan semi-automatic bacterial identification system. The drug susceptibility was detected by the drug susceptibility analysis system microdilution susceptibility testing. The drug resistances of 64 patients infected with multi-drug resistant pseudomonas aeruginosa in ICU of our hospital from January 2008 to December 2011 were analyzed statistically. Results The multi -drug resistant pseudomonas aeruginosa in 64 patients was sensitive to quinolones: the drug resistance rate was 21.9% for Ciprofloxacin and 25.0% for Levofloxacin. Their drug resistance rates for penicillins and cephalosporins were high: 100.0% for Penbritin, Penbritin/sulbactam, Cefazolin, Cefoxitin, 98.4% for Cefotaxime, 90.6% for Ceftriaxone. Their drug resistance rate for aminoglycosides showed that 40.6% for Gentamicin, 32.8% for Amikacin and 32.8% for Tobramycin respectively. Their drug resistance rate for β-lactamase/β-lactamase inhibitors showed that 100.0% for Amoxicillin/Clavulanic Acid, 46.8% for Ticarcillin/Clavulanic Acid and 29.7% for Piperacillin/Tazobactam respectively. Their drug resistance rate for the representative drug of monocyclic class amide (Aztreonam) was 45.3%, and the drug resistance rate for the representative drug of carbapenem (Imipenem) was 32.8%. Conclusion Pseudomonas aeruginosa in ICU of our hospital has multidrug resistance, which should be treated with drugs according to medicine sensitive experiment results. For pan-drug resistant bacterial strains, Polymyxin B can be chosen to resist infection.%目的 探讨医院重症监护病房(ICU)多重耐药的铜绿假单胞菌对常用抗生素的耐药性,指导临床合理用药.方法 采用Microscan-autoscan半自动细菌鉴定系统鉴定菌株,以药敏分析系统微量稀释法检测药物敏感性,对2008年1月~2011年12月入住我院ICU发生院内多重耐药铜绿假单胞感染的64例患者的药物耐药性进行统计分析.结果 64例多重耐药铜绿假单胞菌显示对喹诺酮类药物较敏感:环丙沙星耐药率为21.9%,左氟沙星耐药率为25.0%;对青霉素类和头孢类耐药率较高:氨苄青霉素、氨苄青霉素/青霉烷砜、头孢唑林、头孢西丁耐药率均为100.0%,头孢噻肟为98.4%,头孢三嗪为90.6%;对氨基糖苷类药物耐药情况:庆大霉素耐药率为40.6%,丁胺卡那霉素为32.8%,妥布霉素为32.8%;对β内酰胺酶/β内酰胺酶抑制剂耐药情况:阿莫西林/克拉维酸耐药率为100.0%,替卡西林/克拉维酸为46.8%,哌拉西林/他唑巴坦为29.7%;对单环内酰胺类代表性药物氨曲南的耐药率为45.3%;对青霉烯类代表性药物亚胺培南的耐药率为32.8%.结论 我院ICU的铜绿假单胞菌多具有多重耐药性,对多重耐药的铜绿假单胞菌抗感染用药可以根据药敏结果选择,对于泛耐药菌株则可以选择多黏菌素B抗感染.
