腹膜透析患者首次发生腹膜炎的危险因素研究

Objective To investigate the influencing factors of first-onset peritonitis in peritoneal dialysis patients.Methods We enrolled 213 patients who underwent peritoneal dialysis catheter insertion and maintenance peritoneal dialysis from 2013 to 2015 in the Nephrology Hospital,the First Affiliated Hospital of Zhengzhou University.Age,gender,body mass index(BMI),smoking status,history of hypertension and diabetes,serum albumin level at the beginning of peritoneal dialysis,and peritoneal transport function were retrieved from their medical records.Follow-up began at the time of peritoneal dialysis catheter insertion and the initiation of dialysis.The study endpoint was first-onset peritonitis.Follow-up was performed until hemodialysis,renal transplantation,death,or study end(2015-12-31).Results Multivariate Cox proportional hazards regression model analysis showed that gender〔HR=2.566,95％CI(1.421,4.635)〕,BMI〔HR=1.954,95％CI (1.220,3.128)〕,smoking status〔HR=1.659,95％CI(1.052,2.618)〕,and serum albumin level〔HR=2.024,95％CI (1.246,3.286)〕 were independently associated with first-onset peritonitis(P<0.05).Conclusion Male peritoneal dialysis patients with a high BMI,a smoking history,and hypoalbuminemia were at increased risk of developing first-onset peritonitis.%目的 探讨接受腹膜透析(PD)的患者首次发生腹膜炎的危险因素.方法 选取2013—2015年于郑州大学第一附属医院肾脏病医院接受PD置管术并开始维持性PD的213例患者为研究对象.收集患者性别、年龄、体质指数、吸烟状态、有无高血压和糖尿病、开始PD时血清清蛋白水平,以及腹膜转运功能等临床资料.以患者接受PD置管术并开始维持性PD为随访起点,结局事件为首次发生腹膜炎,随访至患者转为血液透析、肾移植、死亡或研究终点(2015-12-31).结果 多因素Cox比例风险回归模型分析显示,性别〔HR=2.566,95％CI(1.421,4.635)〕、体质指数〔HR=1.954,95％CI(1.220,3.128)〕、吸烟状态〔HR=1.659,95％CI(1.052,2.618)〕、血清清蛋白〔HR=2.024,95％CI(1.246,3.286)〕是PD患者首次发生腹膜炎的独立影响因素(P<0.05).结论 男性、高体质指数、有吸烟史、低清蛋白血症的PD患者发生腹膜炎的风险增加.
