异丙酚-瑞芬太尼不同输注方式在颅内动脉瘤介入治疗中的应用

         

摘要

Objective To investigate the effects of different infusion methods of remifentanil and propofol in intracranial aneurysm embolization surgery. Methods 65 ASA II -Ⅲ patients undergoing intracranial aneurysm intervention surgery were randomly divided into two groups: continued infusion group (A group) and target-controlled infusion group ( B group) . Time of exposing in the X-ray of anesthesist, time of extratubation after surgery, 0AAS score of patients on different times and dosage of propofol and remifentanil were recorded. Results Exposing time of the X-ray of anesthesist in group A was obviously shorter than group B. There were no differences between the two group in time of extratubation and OAAS score and anesthesia duration. Dosage of propofol in group A was less than group B,but the dose of remifentanil was same. Conclusion Target-controlled infusion of propofol and remifentanil is more suitable for intracranial aneurysm embolization surgery.%目的 比较异丙酚和瑞芬太尼不同输注方法在颅内动脉瘤介入手术麻醉中的临床应用效果.方法 ASA Ⅰ~Ⅱ级择期行颅内动脉瘤介入治疗的患者65例,随机分为两组,A组32例,微量泵持续恒速输注异丙酚和瑞芬太尼;B组33例,采用血浆靶控输注模式.观察两组麻醉医师暴露在放射线下的时间、术后拔管时间,患者拔管后即刻和拔管后15 min、30 min的意识状态(OAAS),麻醉时间及异丙酚和瑞芬太尼的用量.结果 B组手术中麻醉医师暴露在放射线下的时间明显少于A组(P<0.05);A、B两组患者拔管时间和拔管后即刻及拔管后15 min、30 min的意识状态评分无显著差异(P>0.05);麻醉时间两组无明显差异,异丙酚用量A组少B组,瑞芬太尼用量无明显差异.结论 异丙酚-瑞芬太尼靶控输注更适合颅内动脉瘤的介入治疗.
