Objectives:To observe the clinical value and prognosis of modified B-Lynch suture in treating intraoperative hemorrhage of cesarean section.Methods:80 patients with intraoperative hemorrhage in our hospital from February 2014 to February 2015 were selected and according to the different methods of hemostasis were divided into the study group and the control group,each of 40 patients.The study group was treated by modified B-Lynch suture for hemostasis,while the control group was treated with intrauterine packing with gauze hemostasis.The operative time,antibiotic use time and average hospitalization time and the amount of bleeding during surgery and related cases of patients with postpartum sex hormone level and blood transfusion during surgery,postoperative vaginal bleeding,changes in Hb,the rate of postpartum hemorrhage,postoperative complications and efficiency were all recorded and compared.Results:There were no significant differences between the two groups in operation time,antibiotic use time and hospitalization time (P ＞ 0.05).The intraoperative blood loss had no significant difference between the two groups (P ＞ 0.05).Difference in the postoperative FSH,LH,E2 and other sex hormone levels between the two groups was not statistically significant (P ＞ 0.05).The intraoperative blood transfusion and vaginal bleeding volume and changes in Hb in the study group were significantly lower than the control group,with statistical significant difference (P ＜ 0.05).The incidence of postpartum hemorrhage in the study group was 2.5％,significantly lower than the control group of 20％,with statistical significance (P ＜ 0.05).The incidence of complication in the study group and control group was 5％ and 20％ respectively,with statistically significant difference (P ＜0.05).The total effective rate in the study group and control group was 90％ and 72.5％ respectively,with statistical significance (P ＜ 0.05).Conclusion:The modified B-Lynch suture is of remarkable curative effect for treatment of intraoperative hemorrhage in cesarean section,with high success rate and rapid hemostasis effect,which is worthy promotion in clincial.%目的:研究改良式B-Lynch缝合术治疗剖宫产术中出血的临床应用价值及预后观察.方法:选取我院2014年2月至2015年2月期间接诊的80例进行剖宫产术而出血的产妇作为研究对象,根据产妇不同的止血方式对其进行分组,每组40人,其中研究组患者采用改良式B-Lynch缝合术进行止血,对照组患者采用宫腔填塞纱布进行止血,检测并记录所有患者的手术时间、应用抗生素时间、平均住院时间、术中出血量等手术相关情况和患者产后性激素水平、手术过程中的输血量、术后阴道出血量、手术前后Hb变化量、产后出血率、手术后并发症发生率以及手术后有效率等,并对其结果进行比较分析.结果:两组患者的手术时间、应用抗生素时间、平均住院时间差异无统计学意义(P＞0.05);在手术过程中,两组患者的术中出血量差异无统计学意义(P＞0.05);在剖宫产手术后,两组患者的FSH、LH、E2等性激素水平均没有明显的差异(P＞0.05);而研究组在手术过程中的输血量、术后阴道出血量以及手术前后Hb变化量均明显低于对照组,差异具有统计学意义(P＜0.05),研究组产后出血率为2.5％,明显低于对照组20.0％的产后出血率,差异显著(P＜0.05);研究组并发症总发生率为5.0％,明显低于对照组20.0％的并发症总发生率,差异具有统计学意义(P＜0.05);研究组总有效率为90.0％,明显高于对照组患者72.5％的总有效率,差异具有统计学意义(P＜0.05).结论:改良式B-Lynch缝合术治疗剖宫产术中出血疗效显著,具有操作过程简单、产后并发症发生率低、成功率高、止血迅速有效等特点,具有临床应用价值,值得推广.
