诺卡菌耐药性及氟喹诺酮类耐药相关基因研究

摘要

Objective To investigate the species, antimicrobial resistance, and fluoroquinolone resistance gene profiles of Nocardia in the General Hospital of Ningxia Medical University. Methods Nocardia isolates were collected in the General Hospital of Ningxia Medical University during the period from January 2013 to June 2017. Broth dilution method was used to determine the susceptibility of the Nocardia strains to 15 antibiotics. Fluoroquinolone resistance genes gyrA, parC, qnrA, qnrB and qnrS were detected by polymerase chain reaction. Results A total of 16 isolates of Nocardia were collected, including Nocardia cyriacigeorgica (62.6％, 10/16), Nocardia farcinica (18.7％, 3/16), and Nocardia otitidiscaviarum (18.7％, 3/16). Most Nocardia isolates (81.2％, 13/16) were resistant to ciprofloxacin. About 12.5％, 12.5％, 25.0％ and 18.7％ of the strains were resistant to ceftriaxone, tobramycin, gentamicin and cefepime. Overall, gyrA, parC, qnrA and qnrB were identified in 18.8％, 37.5％, 25.0％ and 18.7％ of the Nocardia strains. The qnrS gene was not found in any strain. Conclusions The most frequently Nocardia species in the hospital was Nocardia cyriacigeorgica. Fluoroquinolone resistance is serious in the Nocardia isolates.Fluoroquinolone resistance of Nocardia is likely associated with gyrA, parC, qnrA, and qnrB genes. We should pay close attention to the emergence and antimicrobial susceptibility of Nocardia infection.%目的 了解宁夏医科大学总医院临床分离诺卡菌的种类、药敏特点及对氟喹诺酮类耐药基因的分布.方法 收集2013年1月-2017年6月临床分离的16株诺卡菌,使用微量肉汤稀释法检测其对15种抗菌药物的药物敏感性,并检测对喹诺酮类耐药相关基因gyrA、parC、qnrA、qnrB、qnrS.结果 16株诺卡菌中,盖尔森基兴诺卡菌最多见,占62.6％(10/16),其次为鼻疽诺卡菌和豚鼠耳炎诺卡菌,各占18.7％(3/16).其对环丙沙星耐药率达到了81.2％,对头孢曲松、妥布霉素、庆大霉素、头孢吡肟耐药率分别为12.5％、12.5％、25.0％、18.7％.16株诺卡菌中,gyrA、parC、qnrA、qnrB检出率分别为18.8％、37.5％、 25.0％和 18.8％,未扩增出qnrS基因阳性结果.结论 该院诺卡菌最多见的菌种为盖尔森基兴诺卡菌,菌株对氟喹诺酮类药物耐药严重,耐药机制可能与gyrA、parC、qnrA、qnrB基因有关.应密切注意当地诺卡菌在临床上的发生和重视药敏试验.
