Objectives To evaluate the subcellular localization of glucocorticoid receptors α (GRα) in PBMCs and its clinical significance in children with idiopathic thrombocytopenic purpura (ITP). Methods Glucocorticoid receptor α (GRa) subcellular localization was examined by confocal microscopy. Protein phosphorylation was measured by means of Western blot. Results The GRα of sensitive group localized in nucleus, but in the insensitive group the GR localized in cytoplasma after dexamethasone treatment. The expression of GRα showed no significant difference among the groups. Conclusion The abrogation of GRα nuclear translocation may serve as a novel predictor of glucocorticoid resistance in patients with ITP.%目的 探讨糖皮质激素受体α(GRα)在儿童特发性血小板减少性紫癜(ITP)外周血单个核细胞的亚细胞分布及临床意义.方法 选择2009年6月至2011年9月在淄博市中心医院儿科住院的ITP患儿24例,按临床疗效分为两组:激素有效组(14例)和激素无效组(10例).停激素2周后抽外周血分离单个核细胞,加入地塞米松1h后,采用间接免疫荧光与激光共聚焦显微镜技术检测ITP患儿外周血单个核细胞GRα的亚细胞定位,并计算核浆比.Western blot检测GRα的表达.结果 激素有效组ITP患儿GRα趋向分布于胞核,而激素无效组GRα主要分布在胞浆,核浆比分别为1.43±0.07,0.96±0.03,差异有统计学意义(P＜0.05).两组GRα的表达分别为1.13±0.73,1.43±0.07,差异无统计学意义(P＞0.05).结论 ITP患儿激素无效可能与糖皮质激素受体α的核转位障碍有关.
