目的：通过比较抗CCP抗体、AKA、APF、抗MCV抗体和RF诊断类风湿性关节炎（rheumatoid arthritis,RA）的敏感性和特异性，探讨多参数检测早期诊断RA的临床意义。方法将调查对象分为RA患者组、非RA自身免疫疾病组和健康组3组，进行抗CCP抗体、AKA、APF、抗MCV抗体和RF检测，统计3组的差异性。结果在RA患者组中，抗CCP抗体、AKA、APF、抗MCV抗体和RF的检出率分别为67.39%、47.83%、51.86%、38.51和80.12%；非RA自身免疫疾病组检出率分别为1.90%、0%、1.90%、8.57%和22.95%；健康组检出率分别为0%、0%、0%、0%和8%；在RA患者组中，抗CCP抗体、AKA、APF、抗MCV抗体、RF和5项联合检测的敏感性分别为81.02%、55.12%、33.75%、72.08%、80.07%和95.22，特异性分别为96.15%、92.32%、98.68%、95.48%、76.85%和84.51%。结论通过5项抗体联合检测，对RA的早期诊断具有重要的临床意义。%Objective Through the calculation and comparison of anti CCP antibody, AKA, APF, anti MCV antibody and RF diagnosis, the sensitivity and speciifcity of RA, to investigate the clinical signiifcance of multi parameter detection in the early diagnosis of RA.Methods Of multi subjects were divided into RA group, RA in patients with non autoimmune diseases and healthy group three groups, are anti CCP antibody, AKA, APF, anti MCV antibody and RF detection, the difference of statistics and calculation of each.Results In RA group, the detection of anti CCP antibody, AKA, APF, anti MCV antibody and RF were 67.39%, 47.83%, 51.86%, 38.51 and 80.12%. RA group of non autoimmune disease detection rates were 1.90%, 0%, 1.90%, 8.57% and 22.95%. Health detection rates were 0%, 0%, 0%, 0% and 8%; in RA group, the sensitivity of anti CCP antibody, AKA, APF, anti MCV antibody, RF and combined detection of ifve were 81.02%, 55.12%, 33.75%, 72.08%, 80.07% and 95.22%, speciifcity was 96.15%, 92.32% 98.68%, 76.85%, 95.48%, and 84.51%.Conclusion The combined detection of ifve kinds of antibodies, is of clinical signiifcance for the early diagnosis of RA.
