目的 对中国核工业北京四O一医院近三年来慢性阻塞性肺疾病(COPD)患者进行预后调查分析,并评估综合性护理干预在其中发挥的效果.方法 选择2014年6月至2017年6月住院治疗的176例COPD患者并随机分为对照组(n=88)和观察组(n=88).在接受临床基准治疗的基础上,对照组患者实施常规护理干预,观察组患者实施综合性护理干预.在后期随访时间内,分别比较2组患者出院后随访结局、预后效果及生活质量.结果 观察组患者遵医嘱用药、家庭氧疗和呼吸锻炼执行的比例均显著高于对照组(P<0.05);观察组患者咳喘脓痰症状缓解率、半年内复发率及FEV1和FEV1/FVC指标情况均显著优于对照组(P<0.05);观察组患者SF-36生活质量评分也显著优于对照组(P<0.05).结论 针对COPD患者运用综合性护理干预可有效提高患者的治依从性及症状缓解率,在切实改善COPD患者肺功能的基础上进一步提高了患者的生活质量,对疾病康复具有良好效果.%Objective To analyze the prognosis of patients with chronic obstructive pulmonary disease ( COPD) in our hospital in last three years, and to evaluate the effects of comprehensive nursing intervention. Methods A total of 176 patients with COPD who were hospitalized and treated in our hospital from June 2014 to June 2017 were randomly divided into control group (n=88) and observation group (n=88). On the basis of clinical basic treatment,the patients in control group were treated by routine nursing intervention,however, the patients in observation group were trreated by comprehensive nursing intervention. The follow up outcome,prognosis and life quality of patients were observed and compared between two groups. Results The proportion of patients with good compliance of medication under supervision of a physician, family oxygen therapy and respiratory exercise in observation group was significantly higher than that in control group (P<0. 05). Moreover the remission rate of cough expectoration symptoms,recurrence rate in half a year,FEV1 and FEV1/FVC in observation group were superior to those in control group (P<0. 05). The life quality score of sf-36 in observation group were significantly higher than those in control group (P<0. 05). Conclusion The patients with COPD who are treated by comprehensive nursing intervention can improve patient' s compliance and symptom remission rate,on the basis of practical improvement of lung function of patients with COPD,which can improve further the life quality of patients.
