目的：探讨异基因造血干细胞移植（ allogeneic hematopoietic stem cell transplantation ， allo-HSCT）受者真菌性肺炎的发病时间CT表现及动态发展变化。方法回顾性分析28例allo-HSCT受者真菌性肺炎的CT表现及临床资料。结果28例真菌性肺炎，发生在移植后30天内4例，31～100天8例，101～365天6例，365天以后10例。24例CT主要表现为结节和（或）团块状影，其中7例伴晕征；16例病程中出现空洞，其中11例空洞出现分隔征；11例出现气腔实变；10例出现磨玻璃密度；6例出现局限性支气管扩张；6例出现网织状改变。经抗真菌治疗后，17例病变吸收，其中10例患者病变吸收过程中出现空洞伴分隔征。结论 allo-HSCT受者真菌性肺炎可发生在移植后各个时间段，其CT表现多种多样，主要表现为结节、团块及空洞；空洞伴分隔症可能是病变吸收的表现。%Objective To investigate the CT findings of fungal pneumonia in allogeneic hematopoietic stem cell transplantation recipients.Methods The CT findings and clinical data of fungal pneumonia in 28 allo-HSCT recipients were studied retrospec-tively .Results Among the 28 cases of fungal pneumonia , 4 cases occurred within 30 days after transplantation , 8 cases occurred within 31 to 100 days, 6 cases occurred within 101 to 365 days, 10 cases occurred after 365 days.24 cases showed nodules or masses, 7 nodules were surrounded with halo sign;there were 25 cavitations in the course of fungal pneumonia in 16 patients, 12 cavitations had septation pattern;10 cases showed air-space consolidation;10 cases showed ground-glass opacification;6 cases showed bronchial dilatation and bronchial wall thickening;6 cases showed reticular changes .After the anti-fungal treatment , 17 cases were absorpt , 10 cases of lesion evolved into cavitations with septation pattern during absorption .Conclusion The CT find-ings of fungal pneumonia in allo-HSCT recipients are various ,the most frequent CT findings were nodules , masses and cavitations , the cavity with septation pattern may be the CT features during absorption of lesion .
