糖尿病护理门诊患者失访原因分析与对策

         

摘要

Objective To investigate the reasons of loss to follow-up of type 2 diabetes outpatients and provide related suggestions. Methods With retrospective analysis, telephone follow-up was implemented in type 2 diabetes patients who lost follow-ups in nursing-led outpatient department. Results There were several main reasons for loss to follow-up including poor awareness of return visit to diabetic nursing-led outpatient department (40.1%); medical insurance (16.5%); aging, lying in bed and difficulty in walking (11.3%) and long distance (13.7%). Conclusion The awareness of return visit should be improved further. Targeted follow-up approaches like home visit and telephone follow-up should be implemented separately and as a result patients ' self-management competence could be improved.%目的 分析糖尿病护理门诊患者失访原因并提出相应对策.方法 采用电话随访的方式,对失访的395例2型糖尿病患者采用开放式提问以明确患者失访的原因.结果 共与 212例(53.7%)患者取得联系,电话随访后发现患者未按时到糖尿病护理门诊复诊的原因主要有:患者缺乏到糖尿病护理门诊复诊的意识(占40.1%);医疗定点单位不在建档医院(占16.5%);患者自我感觉良好,认为没必要到糖尿病护理门诊复诊(占14.2%);患者年龄大、卧床及行走不便(占11.3%);离建档医院较远,在附近医院就诊(占13.7%).结论 需进一步提高患者到糖尿病专科护理门诊进行定期复诊的意识,采取针对性的策略,加强对糖尿病护理门诊的宣传力度,从而加深人们对糖尿病护理门诊的认识;加强对糖尿病患者相关知识宣教,提高2型糖尿病患者的自我管理行为.
