慢性心力衰竭利尿剂联合应用的临床疗效探讨

摘要

目的 探讨利尿剂联合应用在慢性心力衰竭患者利尿剂反应性方面的影响.方法 将符合纳入标准的慢性心力衰竭患者78例随机分为2组,利尿剂常规组(n=42)给予日常口服利尿剂总量的1倍(静脉&口服)治疗.联合组(n=36)口服利尿剂(静脉&口服)+氢氯噻嗪25mg(口服,1次/d)+乙酰唑胺250mg(口服,3次/d).观察两组患者半年内利尿剂抵抗发生率、60d再入院率及其他生化指标间的差异.结果 住院期间利尿剂联合组日均利尿剂总量明显少于常规组(P0.05).结论 慢性心力衰竭患者利尿剂联合应用方案可不同程度改善利尿效果,降低心力衰竭再入院发生率.%Objective To?explore?the?effects?of?combined?use?of?diuretics?in?chronic?heart?failure?patients.?Methods 78?chronic?heart?failure?patients?met?the?inclusion?criteria?were?randomly?divided?into?routine?diuretic?group(n=42)and?combined?use?of?diuretic?group(n=36).?Patients?in?routine?diuretic?group?were?given?1times?dose?of?daily?oral?diuretic?and?patients?in?the?other?group?were?given?daily?oral?diuretic,hydrochlorothiazide (25mg?per?day)and?acetazolamide(250mg?three?times?a?day).Incidence?rate?of?diuretic?resistance,readmission?rate?in?60?days?and?side?effects?of?diuretic?in?the?2?groups?were?observed.?Results The?daily?dose?of?diuretics?in?the?combined?use?of?diuretic?group?was?significantly?less?than?routine?diuretic?group(P0.05).Conclusions Loop?diuretic?combined?hydrochlorothiazide?and?acetazolamide?can?improve?the?effect?of?diuretics?in?chronic?heart?failure?while?the?side?effects?of?diuretics?are?not?increased.
