The February 2009 Colgan Air Q400 crash that killed 50 people, the only fatal commercial airline accident in the US in the last five years, continues to reverberate for regional airlines. The US National Transportation Safety Board (NTSB) identified pilot error as the cause of flight 3407's demise, citing the captain's inappropriate response to a stall warning. An issue that received a great deal of attention during Congressional hearings that followed the accident was whether the certification requirements for Part 121 first officers are sufficient. The 24-year-old first officer in the doomed Q400 had a commercial pilot certificate but not an Airline Transport Pilot (ATP) certificate.
