Primary Cutaneous Nocardiosis of Axillary Region: A Case Report

Background: Nocardiosis is an uncommon butworld-wide infection caused by several speciesof soil-borne aerobic bacteria belonging to thegenus Nocardia. Primary cutaneous nocardio-sis (PCN) is an uncommon entity. It usuallyoccurs among immunocompetent but occupa-tionally predisposed individuals. Clinically, itcan present as acute infection (abscess or cel-lulitis), mycetoma, or sporotrichoid infection[1]. Here we are reporting a case of PCN pre-sented as mycetoma in axilla which is a raresite. Case History: The patient had extensivelesions in and around the axilla, which could beattributed to the fact that the patient, being anagriculturist, had been exposed to recurrenttrauma while carrying firewood and soiledsacks. Single lesion initiated four years ago,progressed to multiple lesions with few healedscars. Despite the treatment in several hospi-tals, lesions recurred. The present patient wasdiagnosed as PCN caused by Nocardiabrasiliensis and appropriately treated. Conclu-sion: Nocardia infection should be consideredin the differential diagnosis of a supportive andgranulomatous dermatitis that presents clini-cally as multiple discharging sinuses with pap-ules and nodules in and around axilla apart fromtuberculosis.
