There is no firm evidence that intranasal fentanyl is more effective than buccal fenfany/. The packaging is not entirely safe, thus exposing patients and others to major risks.Short-acting oral morphine is the standard treatment for breakthrough cancer pain despite adequate maintenance opioid therapy (1). Other useful options include three forms of buccal fentanyl, mainly gingival "lollipops" (tablets on a stick) (1-3).Fentanyl nasal spray is now authorised for use in this setting (Instanyl, Nycomed). In practice, is breakthrough pain relieved more rapidly or more effectively with fentanyl nasal spray than with buccal fentanyl? Is intranasal administration more appropriate for some patients? And how safe is the packaging?
