The role of surgery in the treatment of asymptomatic hyperparathyroidism has been controversial. It has been argued that most patients are not truly asymptomatic and will acknowledge symptoms that can be attributed to hyperparathyroidism when questioned in a focused manner. The Consensus Development Conference on the Management of Asymptomatic Primary Hyperparathyroidism was held at the NIH in 1990 and made recommendations regarding the indications for surgery in asymptomatic patients, and these recommendations were recently updated in a similar workshop. This conference and follow-up workshop addressed only the classic signs and symptoms of primary hyperparathyroidism and considered more nonspecific nonclassic symptoms such as fatigue, irritability, and mood swings as asymptomatic.
